FY 2027 Hospice Aggregate CAP: What Leadership Should Monitor Every Month

The aggregate cap is easy to treat as a year-end calculation, because year-end is when the final exposure becomes visible. Operationally that is far too late. By the time a cap liability is confirmed, every decision that produced it has already been made.

Hospice leaders need an estimate that moves with census, length of stay, admissions and Medicare revenue throughout the cap year.

Know the current cap year and amount

CMS set the FY 2027 aggregate cap amount at $36,174.75, up from $35,361.44 in FY 2026. The 2027 cap year runs from October 1, 2026 through September 30, 2027.

The cap limits total Medicare payments to the hospice based on the allowable cap amount multiplied by the number of beneficiaries under the applicable calculation. Both halves of that formula move during the year, which is exactly why a single annual look is not enough.

Track projected exposure, not just actual payments

A useful monthly view estimates where the hospice is heading if current census and length-of-stay patterns continue, rather than reporting where it has been.

Waiting for a final cap report converts a manageable trend into an unpleasant surprise, usually at the worst possible point in the fiscal year.

Watch length of stay and patient mix

Cap exposure gets discussed as a billing problem, but operations create most of the risk. Long lengths of stay, shifting admission patterns and census growth can move the estimate quickly.

Finance, intake and clinical leadership should be looking at the same trend line. When they are not, the conversation turns into a disagreement about numbers instead of a decision about admissions.

Reconcile the data sources

If the census report, the billing system and the finance model use different definitions or different dates, leadership will spend meetings arguing about which number is right.

Document which source drives the estimate, how often it updates, and who validates anomalies. That one page prevents most of the arguing.

Use scenarios instead of a single forecast

Build three views: a base case, a higher-census case and a longer-length-of-stay case.

Scenario modeling answers the question leadership actually needs answered, which is not “what is our number” but “how much normal variation can we absorb before this becomes a problem”.

Make CAP a standing leadership metric

The cap estimate belongs next to AR, census, admissions and cash, not in a spreadsheet that gets opened once a year.

Monthly visibility supports better planning and, more usefully, lets leadership see the financial meaning of operational trends while there is still time to respond to them.

Financial and compliance note: Cap calculations are complex and depend on current CMS methodology and beneficiary counts. Validate organization-specific calculations against current CMS and MAC guidance.

Pfundamental provides hospice financial review, forecasting and cap estimates. If your team is still treating the cap as an annual surprise, a monthly monitoring model is a practical place to start.
