iQIES Review and Correct Reports: A Monthly Hospice Quality Routine

Submitting HOPE records is not the same as knowing what your quality data will show.

CMS gives hospices iQIES reports that can be used to review quality-measure data and identify records that may need attention before a public-reporting correction deadline. Most organizations have access to these reports and open them twice a year, usually in a hurry.

Know what the Review and Correct report is for

CMS describes the Hospice Review and Correct Report as a way to review assessment-based quality-measure data before the quarter data submission deadline.

The report shows rolling-quarter information and indicates whether the correction period is still open or has closed. That second detail is the one teams tend to discover too late.

Put the report on a calendar

If the organization opens the report only when a deadline is close, there is rarely enough time to investigate records, correct data and confirm the corrected submission.

A monthly review creates runway. It also makes unusual trends visible while they are still small enough to explain.

Assign an owner and a backup

Quality, clinical informatics or compliance may own the process, but the ownership should be written down rather than assumed.

The backup matters more than it sounds like it should, because reporting deadlines do not move when the primary person is on vacation or leaves the organization.

Investigate the patient-level story

When a measure moves unexpectedly, use the available patient-level quality reports to find which records contributed to the result.

The goal is not to improve the score by editing data. It is to confirm that the accepted data accurately reflects the clinical record. Those two activities can look similar from the outside and are entirely different in substance.

Respect the public-reporting correction window

CMS sets a defined correction period for each Compare refresh. Changes made after the relevant deadline can affect later reporting but will not change the current refresh.

Teams should know the key dates before the quarter closes, not after. CMS publishes them, and they belong on the same calendar as the monthly review.

Bring the findings into QAPI

If recurring errors trace back to timing, staff interpretation, workflow design or software behavior, the issue belongs in process improvement rather than in a correction queue.

Quality reporting earns its keep when it reveals where the real process needs attention, which is also the only version of it that reduces work over time.

Compliance note: iQIES reports, measures and deadlines can change. Use the current CMS HQRP key-date pages and report manuals for each reporting period.

Pfundamental can help hospices turn iQIES review into a routine quality-control process instead of a deadline-driven scramble.
